Martin Luther King Jr (MLK) Observance: COMMUNITY LEADERSHIP NOMINATION FORM

The purpose of the Martin Luther King Jr (MLK) Observance is to acknowledge, celebrate and embrace Dr. King’s legacy of service and support for individuals of all backgrounds. The annual event recognizes that we all share a common goal of educating the leaders of tomorrow and the belief that there is importance in turning community concerns into citizen’s actions of service.

The Community Leadership Award recognizes an individual from a business, group, agency, organization or community initiative that exemplify a commitment to service and inclusion throughout their communities.

The following information is required:

PROVIDE THE FOLLOWING CONTACT INFORMATION OF THE PERSON BEING NOMINATED:
ESSAY REQUIREMENTS:
  • Brief bio of nominee:
  • Provide evidence that the nominee has a sustained commitment to service within their community and/or region.
  • Provide evidence that nominee has instilled the value of service and inclusion in others.
  • Discuss the impact of nominee’s acts of service and inclusive practices within the community.
  • Provide additional information to support nomination.
ESSAY NOMINATION SUGGESTIONS:
  • Review all information and criteria
  • Address each area in detail and avoid general statements
  • Provide the necessary information for each question to qualify and strengthen your nomination (If your nominee has been involved in several initiatives, make sure to provide commentary on each to strengthen your nomination.)
  • Remember, this award will be based on the strength of your nomination- make sure to take your time!
YOUR ESSAY
NOMINATOR’S CONTACT INFORMATION:

Submit nominations by Friday, December 13, 2019 at 5pm. Nominations will close after deadline date/time.

Please note that an organization must have been in existence for a minimum of three years. Recipients will not be selected based on the number of nominations, but rather the rigor of their efforts. Persons/organizations awarded recognition in the past 3 years are ineligible. Recognition of recipients/organizations will be announced at the Martin Luther King Jr (MLK) Recognition of recipients/organizations will be announced at the Martin Luther King Jr Observance in January 2020.
