Accessibility Services (AS) at Youngstown State University helps students with disabilities receive reasonable accommodations. We review each student's needs individually. To access support, students may be asked to provide documentation of their disability.
What Kind of Documentation is Needed?
We accept several types of documentation. Choose one of the following options:
- Option 1: Completed Documentation Form
- Have a licensed or credentialed professional (such as doctor, psychiatrist, or counselor) fill out our AS Documentation Form
- Option 2: Letter from a Healthcare Provider
- A letter from your healthcare provider should include:
- The name of your disability and date of diagnosis
- How the diagnosis was made (tests or evaluations used)
- How your disability affects you academically
- What major life activities are impacted
- Whether your condition is expected to change or stay the same
- Recommended accommodations (optional)
- Option 3: Diagnostic Report
- You can submit a full evaluation report, such as a :
- Multi-Factored Evaluation (MFE)
- Evaluation Team Report (ETR)
- For learning disabilities, your report should include:
- A test of cognitive ability (preferably normed for adults)
- A test of academic achievement (reading, math, and/or writing)
- Standard scores and percentiles based on your age
- Please Note:
- An IEP (Individualized Education Plan), 504 Plan, or Summary of Performance from high school can help show your history of accommodations, but may not be enough by itself to qualify for services in college.

What Happens Next?
Once we receive your documentation, you will schedule a registration appointment. During this meeting, we will have an interactive discussion to understand your needs and determine appropriate accommodations.
