Petition for Academic Reinstatement - Form

Before you complete this form, determine if you are eligible to petition for reinstatement. Students who have been academically suspended from YSU must remain dismissed for at least one semester before being eligible to petition for reinstatement.  If you meet this criteria, submit this form to petition for academic reinstatement.  The Academic Reinstatement Committee will consider your petition.  Submitting a reinstatement form is not a guarantee of reinstatement to Youngstown State University and/or the program you wish to pursue.  Petitioners will be notified of the Committee's decision.  The Committee's decision is final.

You must submit your petition by the deadline specified above. You may submit your petition as far in advance of the deadline as you wish. Petitions received after the deadline will not be reviewed.

If you have been away from YSU for 3 consecutive semesters, you must apply for readmission before the required closing date for the semester. For more information and the application visit Readmission of Former Students | YSU

Academic Reinstatement Petition Letter

The Academic Reinstatement Petition requires you to write and submit a letter that addresses an explanation of the circumstances that lead to your Academic Suspension, How your circumstances have changed, your academic goals for the upcoming term, and the steps you plan to take to achieve these goals. Academic Recovery Plan Agreement

Agreement to follow the Academic Recovery Plan if reinstated to Youngstown State University. 
Students who are granted reinstatement will receive an academic recovery plan from the Academic Reinstatement Committee and will be required to review this plan with their academic advisor. The plan may include recommendations such as changing your major, completing an academic reassessment, repeating specific classes, participating in tutoring or individual coaching, seeking mental health counseling, or using other strategies designed to support your academic success.
By submitting this form, I acknowledge and agree to follow the academic recovery plan provided to me. I understand that failure to consistently meet the conditions outlined in the plan may affect my future enrollment at YSU.
