Home School Student Application Checklist

As a home school student, you must submit additional paperwork to be considered for admission to our program. Please follow the checklist below. If you have any questions, contact our office at 330-941-2447 or ccp@ysu.edu.

  1. Review the YSU CCP website, including student eligibility and the admission application process.
  2. Apply online to the YSU CCP program by
    1. March 12, 2026, for a summer start;
    2. March 20, 2026, or a fall start;
    3. October 15, 2026, or a spring start.
  3. Email, mail, or drop off the following to the YSU CCP office (email and address below) by the same deadlines above:
    • An official transcript documenting coursework completed in grades 9-12, including 6th 8th-grade coursework if the student is a rising 7th-9th grader.
    • Middle school students must take the Accuplacer Reading Test. Call or email our office to schedule.
    • Submission of ACT or SAT results is optional. Please note: ACT or SAT scores can help with course placement for some classes.
    • A copy of academic assessment reports (i.e. Iowa Basic Skills Test, California Achievement Test, etc.) submitted to the appropriate superintendent of school pursuant to Section 3301-34-04 of the Ohio Administrative Code.
    • A copy of the Superintendent's Exemption Notice showing the student is excused from receiving in-school instruction.
    • Campus Education Plan
    • Financial Responsibility/Parental Consent Form

  • Email application documents to ccpdocs@ysu.edu. Please place in the subject line: CCP Appl Docs - your student's name. 
  • Mail to the following address:  YSU OCAT/CCP Office, 1Tressel Way, Jones Hall, Room 3001
    Youngstown, OH 44555.
  • Drop off the documents at the OCAT/CCP office on campus, Jones Hall, room 3001. Contact our office for directions to our campus location before you make the trip!
