As a home school student, you will need to submit additional paperwork to be considered for admission to our program. Please follow the checklist below. If you have any questions, contact our office at 330-941-2447 or ccp@ysu.edu.
- Review the YSU CCP website, including student eligibility and how to apply for admission.
- Apply online to the YSU CCP program by March 25, 2024.
- Mail or drop off the following to the YSU CCP office (address below) by March 25, 2024:
- Official transcript, which includes overall GPA and ACT scores.
- A copy of the most recent academic assessment reports (i.e., Iowa Basic Skills Test, California Achievement Test, etc.) submitted to the appropriate superintendent of school according to Section 3301-34-04 of the Ohio Administrative Code.
- Curriculum outline detailing course content, textbooks used, and any other relevant information regarding coursework
- Copy of the Superintendent’s Exemption Notice showing the student is excused to receive homeschooling.
- Campus Education Plan
- Financial Responsibility/Parental Consent Form
