Home School Student Application Checklist

Thank you for considering the YSU College Credit Plus Program!


  1. Review the YSU CCP website, including student eligibility and how to apply for admission.
  2. Submit your Letter of Intent (online) to the Ohio Department of Education by April 1.
  3. Apply online to the YSU CCP program by March 26.
  4. Mail, or drop off, the following to the YSU CCP office (address below) by March 26:
    • Official transcript, which includes overall GPA and ACT scores
    • A copy of the most recent academic assessment reports (i.e. Iowa Basic Skills Test, California Achievement Test, etc.) submitted to the appropriate superintendent of school pursuant to Section 3301-34-04 of the Ohio Administrative Code
    • Curriculum outline, detailing course content, textbooks used, and any other relevant information regarding coursework
    • Copy of the Superintendent’s Exemption Notice showing the student is excused to receive home schooling
    • YSU On-Campus/Online Education Plan (form is on our website)
    • YSU Financial Responsibility/Parental Consent Form (form is on our website)
  5. Look for an e-mail or letter from YSU about next steps or placement testing.
  6. If admitted, send a copy of your acceptance letter with your state funding application to the Ohio Department of Education by 5:00 p.m. on April 12.
    • If your COMPLETED application is not received by March 26, there is no guarantee you will receive your acceptance letter to submit by the state's funding deadline of April 12 by 5:00 p.m.
  7. Email a copy of your Ohio Department of Education funding award letter to the YSU CCP office (ccp@ysu.edu) within two days of receipt.
  8. If admitted and approved for registration, plan to come to campus for orientation, academic advisement and registration
