Policy and Procedures for Testing
 

Testing Policy

  • If student is requesting to take a test at a different time than when it is scheduled, student must get the professor’s permission before scheduling.
  • Student must schedule a test a minimum of 72 hours prior to the test date. If your request date is less than 72 hours before, student will need to complete a form in the Accessibility Services Office.  We can’t guarantee we’ll be able to get the test, but we will try our best.
  • If student is more than 15 minutes late, the professor must be called to obtain permission to start late.  Please call 330.941.1372 if you are running late.
  • Accessibility Services has the right to deny services to students who display misconduct     to other students or AS staff.  Any violation of YSU Student Code of Conduct or academic policy will be referred for disciplinary procedures to the Associate Dean of Students and Conduct. 
  • Student is not permitted to leave the AS Office/exam room once the test has started.
  • Testing rooms are monitored and recorded by a closed-circuit video system.

Student Responsibilities (for those who have testing accommodations)

  • Discuss testing plans with each professor after the accommodation letter is sent and remind professor prior to testing date.
  • Student schedules test in Accessibility Services

  • Sign a test taking agreement before each exam.
    • Test taking agreement statement:
      • I will NOT remove any testing materials from the testing room and NOT disclose (in whole or in part) any exam questions or answers to anyone who has not taken this exam. This disclosure includes anything in oral or written form. I also will NOT cheat in any manner while taking this test. If I am seen cheating my test will be immediately taken from me and my professor notified of the situation. I recognize and acknowledge that violation of this agreement could result in disciplinary action including, but not limited to, the following penalties: suspension, expulsion, transcript annotations. (See Code of Conduct; Article IV, Section A and Article VII, Section E)
      • I understand that testing rooms will be monitored and recorded by a closed-circuit video system.
