All incidents, injuries, near-misses, or unsafe conditions, involving YSU employees, students, contractors, or visitors must be reported to the EOHS Department.
WHAT TO REPORT
Reporting through YSU's electronic form is required for:
1. INCIDENT- An unanticipated event that results in an injury, illness, hazardous spill/release, or significant property damage (e.g. fire or flooding).
2. INJURY- An injury that occurs on YSU campus or during a university sponsored activity.
