Claims Reporting

YSU requires timely and proper reporting of incidents and accidents that could become insurance claims so that insurance coverage is not jeopardized in addition to claims being resolved and paid timely (if coverage applies).  All incidents and accidents can be reported to EHS by completing the online form below.  Depending on the situation, additional information may be needed to determine if a claim must be submitted to our brokers.  Questions about this process can be directed to Julie Gentile, Director of EHS. 

  • Third Party Claims

    Youngstown State University is owned and governed by the State of Ohio. Under Ohio law, a Commercial General Liability claim that seeks to impose financial liability upon the state may be asserted only in the proper forum and in accordance with the procedure provided for by the Court of Claims of Ohio per the Ohio Revised Code (2743.03). It is not until a claim has been filed and is pending in the appropriate forum (Court of Claims) that the University and its insurance carrier possess the authority to compromise and settle a claim.

  • Vehicle Claims Reporting

    Claims involving University owned vehicles must be immediately reported to the Environmental Health and Safety office using the Vehicle Accident Report form.

  • Property and Equipment Loss Reporting

    When a loss occurs involving University property it is the departments responsibility to notify the Environmental Health and Safety office.

  • General Liability Claims

    Incidents on YSU property involving bodily injury or property damage must be immediately reported to the Environmental Health and Safety office.
