Policy for Receiving Notes

To facilitate the note taking program for students with disabilities who are permitted note taking accommodations, the following policies have been developed:

  1. The student is responsible for discussing the need for a note taker with their professors. This should be done by the student meeting with their professor, giving them a copy of their accommodation letter, and discussing their accommodations.
  2. The student must contact CSP Disability Services if there is a change in their schedule any time after the first day of classes.
  3. If a note taker is not needed in a class the student should sign a Note Taker Waiver form and/or call CSP Disability Services immediately so the note taker can be reassigned.
  4. The student must notify CSP Disability Services and the note taker if they will miss class for any reason. This includes scheduled absences and illness. If notification is not given notes may not be provided for the class period.
  • The CSP Disability Services office phone number is: 330-941-1372.
  • It is recommended that the student and the note taker exchange phone numbers and e-mail addresses the first day of class.
  1. Having a note taker is an accommodation given to the student to assist in taking notes in class. This requires the student to attend class on a regular basis.
  • If the student misses class 3 days in a semester without notification or two weeks with notification, the note taker will stop going to the class and services will not continue until the student comes to CSP Disability Services and meets with a Coordinator the Assistant Director or Coordinator to discuss resuming note taking services. If there are extenuating circumstances CSP Disability Services will handle absences on a case by case basis.
  • Note takers are instructed to stay in a class 15 minutes for a 50 minute class and 30 minutes for a class longer than 50 minutes. If you are going to be late please let your note taker know.
  • If the student leaves the class before it is over the note taker has also been instructed to leave. (Exceptions may be made due to a student’s disability.)
  1. If a scheduled class is cancelled. The student should make an effort to contact the note taker as soon as possible to let them know about the cancellation.
  2. Students must sign the note takers time sheet after each class to receive the notes from that class.
  3. If a student needs a note taker outside of the regularly scheduled class period, h/she should contact CSP Disability Services at least 2 weeks before the event to schedule a note taker.
