Form Instructions

 

Page 1 - Applicant Information

  • Section 1:  Applicant's Information
    • Fill out with Applicant's Departmental Information.
    • List reason for requested access (optional)
  • Section 2:  Applicant's Banner Finance Status and Acceptable Use Agreement
    • Indicate whether applicant is a New User in Banner Finance, a Current Finance User, or wishes to have his/her Finance access deleted.
    • Applicant must sign page 1 of the form.
  • Section 3System Access, Approvals and Optional Reporting Access
    • Put a check mark for either Finance or Bursar access. 
    • Optional:  Indicate whether you are requesting WebFocus access or BDM (document imaging) access. Note:  most applicants do not receive WebFocus Finance or BDM access. 
    • Make sure the Financial Manager/Department Head reviews pages 1 and 2, and signs page 1 of the form.
  • Section 4:  Bursar Use Only
    • This section is for Bursar's signature if applicable.

 

Page 2 - Finance Information

  • Section 1 Non-Financial Manager:  This section is filled out by non-financial managers such as Administrative Assistants, Secretaries, etc.  These are people who are not responsible for managing the budget of the fund/org.
  • Section 2 Financial Manager:  This section is filled out by Financial Managers.
    • Section 2a:  Fill out if you are replacing the current financial manager of the fund/org.  List the name of the person you are replacing and be sure to list the funds/orgs that you are requesting.
    • Section 2b:  Applicant will be an additional financial manager on the fund/org.  Be sure to list the funds/orgs that you are requesting.
    • Section 2c:  Applicant is requesting a new fund/org that has not had a financial manager.  Be sure to list the funds/orgs that you are requesting.

 

Make sure the Financial Manager/Department Head reviews/approves the information on Page 1 and Page 2, and signs Page 1.  The completed pages can be emailed to mlcastner@ysu.edu or mailed to the Controller's Office, Jones Hall, room 2011.  Questions, call Mary Lou Castner at x1716 or email mlcastner@ysu.edu.
