YSU CCP School Application Day Request Form

School District Administration & CCP Coordinators:

Requests for CCP Application Days are accepted on a first-come, first-served basis. The new student application and continuing student form is online so a computer lab with internet access is needed for students to complete either.

The YSU CCP New Student Application and Continuing Student Form will open on February 1, 2019. It is due March 26, 2019, for non-public school students and April 5 for public school students.

We will do our best to accommodate your top three options; however, sometimes there may be scheduling and/or staffing conflicts. Any questions, please contact the CCP office at 330-941-2447 or ccp@ysu.edu.
