Financial Aid Action Plan Contract

In signing this agreement, I understand that in order to continue to receive Federal Financial Aid, Federal Regulations require that I must demonstrate academic progress as determined through course completion with a grade of C or better, with no grade of NC, AU, I, or W for the term.

I understand that if I fail to make academic progress or if I fail to demonstrate effort to successfully fulfill this contract, I will lose my Federal Financial Aid.

I agree to implement the following plan of action in order to improve my academic progress:

• Meet with my ASC Academic Coach weekly or as determined by my coach.

• Meet with my Academic Advisor at least once during the semester

• Develop and implement an individualized action plan

• Complete learning assessments and apply strategies based on results

• Attend all classes, communicate with faculty and attend office hours as appropriate

• Track and update grades with my academic coach, as needed

• Follow up on referrals to campus resources. (Tutoring, SI, MAC, WC, Advisor, FA, PSC, etc.)

• Review information from our EARLY ALERT system and implement recommendations as indicated

• I acknowledge my understanding of this contract.
