Immunization Requirements

Immunizations Required to Live in University Housing

The following immunizations are required for all students applying to live in University Housing at Youngstown State University.

  • Tdap: One (1) adult dose in the last 10 years

  • Hepatitis B: Three (3) doses of Hepatitis B OR a lab report confirming immunity

  • MMR: Two (2) doses of MMR, with the second one occurring on or after first birthday; OR two (2) doses measles, two (2) doses mumps, and one (1) dose of rubella; OR lab reporting confirming immunity for each (measles, mumps, and rubella)

  • Varicella: Two (2) doses of varicella OR lab report confirming immunity

  • Polio: Four (4) doses of IPV or OPV (only required for those students who will be younger than 18 at the start of the semester)

  • Meningococcal conjugate (ACWY): One (1) dose since age 16

Immunizations for International Students

In addition to the required immunizations listed above, international students must also provide proof of a recent negative Tuberculosis (TB) test or that you have a test once you arrive at YSU for orientation.

Providing Evidence of Immunizations

Incoming students are required to provide evidence of vaccinations through our partner, MedProctor, prior to moving into the residence halls. There is no charge for this service, though you have the option to purchase a membership, which will provide lifetime access to your immunization records.

To use MedProctor’s service:

Step one: Register

  • As part of the housing application, the student is automatically registered for an account with MedProctor, and should receive an email from the site. If the student loses this email, they can go directly to medproctor.com and select “register” to start a new account. You must use your YSU email account in order for us to be able to verify completion.

Step two: Download

  • Fill out any required personal, medical, or insurance information, which will then be auto-populated into the MedProctor form. You will then need to download the required form.

Step three: Complete forms

  • Take the form that you downloaded to your medical provider, and ask them to complete the form. You cannot simply get a copy of your immunizations records- you must use the form provided.

Step four: Upload

  • Log back in to your MedProctor account and upload a copy of your form. You will receive an email confirmation once the forms have been reviewed.

If you experience any difficulty using MedProctor, you may contact MedProctor between 8:00 am and 5:00 pm Monday through Friday. To access their support service, login to their website and look for the icon in the bottom right-hand side of the screen.

You may also email help@medproctor.com with your issue and this will create a ticket for their support agents to take care of you.

Immunizations Waiver

If you would prefer to complete a waiver for any required immunization (for medical, spiritual or reasons of conscious) please complete our Waiver Form and return it to housing@ysu.edu.  Please note that this document needs to be signed in front of a notary if you are utilizing this option.
