Welcome Back Week Event Form

Please complete the following form to submit your Welcome Back Week event for the schedule. Welcome Back Week takes place from Monday, January 13, 2020 - Saturday, January 18, 2020. The theme for this year's events is Welcome Back Week: 2020 Vision, and coincides with the new year encouraging students to set positive goals for the new semester.

Fields that are marked * are required to complete the submission. The deadline for event submissions is 5:00 p.m. on Wednesday, December 4, 2019. Please double check your submissions to ensure accurate contact information, spelling and grammar. In order to meet marketing material deadlines, no late submissions will be accepted.

Your department or organization is responsible for submitting a reservation request to the appropriate campus reservationist for the location of all programs.
