Program Evaluation

Mahoning County Health District: The Pathways HUB: The Center of Human Services Development provides a process evaluation of the Certified Pathways Community HUB Model Expansion/Replication Infant Mortality Grant. The Mahoning County District Board of Health serves as the administrator and is funded through the Ohio Commission on Minority Health. The purpose of the project is to provide needed clinical health and social services through a network of community partners to meet the needs of at-risk pregnant women in Mahoning County. The Center for Human Services Development works to guide the partners to provide the necessary services and meet the goals stated in the grant proposal. In addition, the Center evaluates and monitors the ongoing operation and accomplishments of the program.

 

Youngstown City Health District: Youngstown Office on Minority Health: The Center has provided a process evaluation for the Youngstown Local Office on Minority Health, housed in the Youngstown City Health District since 2009. The Local Office on Minority Health is funded through the Ohio Commission on Minority Health. The Center evaluates and monitors the ongoing operation and accomplishments and works closely with the Director to provide assistance, as well as make recommendations for program improvement. The Center for Human Services Development meet regularly with the Director and reports quarterly on the community wide activities of the Local Office on Minority Health.

 

Youngstown City School District: Alternative Education Project: The Center delivers an evaluation to assess the progress of the Youngstown City School District’s Alternative Education Challenge Grant that is funded through the Ohio Department of Education. The Center also serves to assist in preparing interim reports for the Ohio Department of Education. These reports include data on the students who participate in the alternative education project. The Center has been working with Youngstown City School District and the Ohio Department of Education on the Alternative Education Project since 2003.

 

Alta Behavioral Healthcare: Classroom Connections: Assessing the effectiveness and impact of the services provided by Alta Behavioral Healthcare to the schools within Youngstown City School District. The Classroom Connections program provides behavioral support to students, teachers, and administration. The Center for Human Services Development evaluates the effectiveness of the program, as well as the impact made on the students, teachers, and administration. An evaluation report is prepared annually discussing the impact of these services provided by Alta Behavioral Healthcare to the Youngstown City School District.
