Getting vaccinated remains the most effective way to battle the virus. We very strongly encourage that unvaccinated students and employees get the vaccine.

Reporting your vaccination

Knowing what percentage of our campus community has been vaccinated against COVID-19 is a critically important piece of information that will better enable YSU to make the best decisions regarding the health and safety of the university community.

We are asking all students, faculty and staff who have been vaccinated to voluntarily share that information.

If you are fully vaccinated or are in the vaccination process, please inform the university of your vaccination status, following the instructions below. 

YSU is utilizing Med+Proctor to collect vaccination information. YSU has a standing relationship with Med+Proctor to retain student immunization records. Med+Proctor is a safe, user-friendly, cloud-based system universities and corporations use to store student and employee immunization records.

Reporting Steps

Make sure that you have a scan or photo of your COVID-19 vaccination card on the device you are going to use to upload them to Med+Proctor. You need to upload a scan or photo as part of the submission process.

  1. Visit the Med+Proctor website. (If you are already logged in, you may skip to step 4.)
  2. Type in your YSU email address

    Type in your YSU email address and click the "Next" button.

  3. You will be re-directed to sign in with your YSU username and password. Sign in.

  4. Verify your information

    Once you sign in, verify your profile information is correct and click the "Continue" button.

  5. Accept the Med+Proctor user agreement

    Read and accept the End User Agreement by entering your full name and clicking the "Agree" button.

  6. Click continue

    After accepting the End User Agreement, click the "Continue" button.

  7. Upload your COVID-19 Vaccine Card
  8. Choose COVID Vaccine in the Document Type drop down

    Choose "COVID Vaccine" in the Document Type drop down and then click "Choose Files" to select the scan or photo of your COVID-19 vaccination card. (JPEG (.jpeg or .jpg) images are preferred.) Click the "Continue" button.

  9. Your record is being reviewed

    Once you have uploaded your record, Med+Proctor will review your record and notify you via email when your submission is approved.

  • Should I get vaccinated?

    Yes. Everyone in the YSU community – faculty, staff and students – are strongly encouraged to get vaccinated. Register for an on-campus vaccine clinic HERE.

  • Am I required to share my vaccination information?

    No. This is a voluntary program, but we encourage you to please participate. Sharing that you have been vaccinated for COVID-19 will provide the university with additional data that will help to inform decisions that will help keep the university safer from COVID-19.

  • What is acceptable proof of COVID-19 vaccination?

    The CDC card issued to you when you received your first COVID-19 shot is the best and easiest way to show proof of vaccination, this is called the COVID-19 Vaccination Record Card. Some healthcare providers/vaccination sites also provided a print-out of your vaccination[s]. This is also an acceptable proof of vaccination. International vaccination verifications will be on a case-by-case basis.

    Please be aware that an employee who submits a fake or fraudulent COVID-19 vaccination card or print-out will be subject to discipline up to and including termination. Additionally, it is a federal crime to create, sell or buy a fraudulent COVID-19 vaccine card with the seal of a U.S. Government Agency.
